Evidence Based Treatment

The Palms focuses on evidence-based psychosocial interventions and uses the evidence-based approach developed by SAMHSA in its manual titled Substance Abuse Treatment For Persons With Co-Occurring Disorders. The facility also uses the Veterans Administration Clinical Practice Guideline for the Management of Patients with SUD. We use the residential levels of care, admission criteria, staffing models, assessment dimensions, intensity, focus of treatment services, and treatment review guidelines found in the Patient Placement Criteria of the American Society of Addiction Medicine (ASAM).

The California Palms divides therapy into eight segments: (1) providing quality addiction therapy with sustainable results, (2) providing long-term mental health treatment, (3) providing vocational assistance to get the veteran back to productive work, (4) integrating family into recovery, (5) incorporating peer support into recovery through Alcoholics Anonymous, Cocaine Anonymous, Narcotics Anonymous and Smart Recovery, (6) making recovery sustainable through physical fitness and good nutrition, (7) indoctrinating positive affirmations, and self-esteem into recovery, and (8) providing sustainable aftercare through connectivity with the Palms staff and facility.

Residential Inpatient Drug and Alcohol Treatment Programs

Soon after admission, each veteran connects with a treatment team that collaboratively develops a strong, customized treatment plan. We offer a number of options for treating substance use and some mental health issues. These options include individual sessions with a therapist, group sessions, wellness groups, education lectures and family engagement, mind-body physical fitness, and other classes that teach how to live full lives while making healthy choices after they leave the facility.

The treatment plan places veterans in the least aggressive treatment environment appropriate to their therapeutic needs. Duration of treatment and treatment options including (1) Medically Managed Detoxification; (2) Residential/Inpatient Care and (3) Aftercare and Relapse Prevention. California Palms 24-hour treatment setting provides a continuum of services that meets the needs of veterans under various levels of care. We adjust the level of care throughout treatment in response to changing clinical needs and treatment goals.

As individuals move through the continuum, they transition to levels of care greater or lesser intensity, depending on the veteran's clinical needs and treatment goals based on the current DSM criteria, and ASAM Patient Placement Criteria. An individual may begin with inpatient treatment only to find that a less intensive level of care, such as intensive outpatient treatment, is more appropriate. Decisions as to which services are provided and how intense is based on the veteran’s needs.

Residential Inpatient Care Program

California Palms offers long term inpatient treatment from drug addiction, alcohol addiction, traumatic stress, PTSD and mental health issues. Following an assessment, a veteran diagnosed with substance abuse problems is provided care in accordance with ASAM Patient Placement Criteria. The duration of inpatient treatment is customized to meet the specific needs of the veteran.

The Palms' residential inpatient program is designed to improve the veteran’s ability to structure and organize the tasks of daily living, stabilize the individual’s substance use symptoms, to develop and apply sufficient recovery skills, and to develop and practice prosocial behaviors such that imminent return to substance use upon transfer to a less intensive level is avoided.

The residential inpatient treatment program is based on evidence-based therapies such as motivation enhancement therapy, 12-step therapist-led counseling, Cognitive Behavioral Therapy, and family involvement. The Palms residential inpatient treatment program has variable lengths of stay. The program includes a range of cognitive, behavioral, and other therapies administered on an individual and group basis; medication management and medication education; counseling and clinical monitoring; random drug screening; planned clinical activities and professional services to develop and apply recovery skills; family therapy; educational groups; occupational and recreational therapies; art, music or movement therapies; physical therapy; and related services.

The Palms' residential inpatient services for co-occurring disorders is appropriate for individuals whose primary problems involve emotional, behavioral, cognitive, readiness to change, relapse, or recovery environment concerns. The California Palms’ residential inpatient care in a 24-hour treatment setting meets the Clinically Managed High-Intensity Residential Services level of care found in ASAM Level 3.5.

Continuum of Care Standards

The 24-hour treatment setting for VA SUD Programs provides a continuum of services capable of meeting the needs of each individual under various levels of care. As clients move throughout the continuum in their recovery they transition to levels of care that are of greater or lesser intensity, depending on their clinical needs and treatment goals. An individual with SUD may begin with intensive inpatient treatment only to find that a less intensive level of care, such as intensive outpatient treatment, is more appropriate.

Our residential inpatient services are designed to improve the patient’s ability to structure and organize the tasks of daily living, stabilize the individual’s substance use disorder symptoms, to develop and apply sufficient recovery skills, and to develop and practice prosocial behaviors such that imminent return to substance use upon transfer to a less intensive level is avoided. The skilled treatment services include a range of cognitive, behavioral and other therapies administered on an individual and group basis; medication management and medication education; counseling and clinical monitoring; random drug screening; planned clinical activities and professional services to develop and apply recovery skills; family therapy; educational groups; occupational and recreational therapies; art, music or movement therapies; physical therapy; and related services.

Comprehensive, multifaceted treatment is provided to individuals with psychological problems, and chaotic or unsupportive interpersonal relationships, and antisocial value systems. The treatment promotes abstinence from chemical dependency, alcohol abuse, opioid addiction, heroin addiction, drug abuse, substance use, and other negative behaviors to effect change in the patients’ lifestyle, attitudes, and values, and focuses on stabilizing current severity and preparation to continue treatment in less intensive levels of care. We transition the levels of care throughout treatment in response to changing clinical needs and treatment goals.

Our residential inpatient services for co-occurring disorders in a 24-hour treatment setting is appropriate for individuals whose primary problems involve emotional, behavioral, cognitive, readiness to change, relapse, or recovery environment concerns. Intoxication, withdrawal, and biomedical concerns, if present, are safely manageable in a 24-hour treatment setting. The American Society of Addiction Medicine (ASAM) industry guidelines are found in Treatment Criteria for Addictive Substance-Related and Co-Occurring Condition. The California Palms’ residential inpatient care in a 24-hour treatment setting provides the level of care found in ASAM Level 3.5 known as Clinically Managed High-Intensity Residential Services. After being assessed, the care is given to a person diagnosed with a substance abuse problem, according to criteria developed by the ASAM using the least restrictive treatment plan possible, depending on specific needs.

Aftercare Program

Upon completion of inpatient/residential or intensive outpatient the veteran will be assessed to determine the level of continuing care required and begin a period of aftercare. Decisions regarding aftercare are based on a continuing care treatment plan identifying specific goals, interventions, and means to assess interventions. A relapse prevention plan will be created and agreed upon prior to discharge.

Upon discharge, veterans are armed with the contacts and resources needed to continue along the road to recovery. Veterans who located outside Northeast Ohio are encourage to affiliate with their local support groups to ensure continuity of treatment. There is no one-size-fits-all aftercare program. We tailor an aftercare program to meet the veterans individual needs. The veteran’s progress will be monitored while in aftercare. The goal is to ensure that the veteran does not need to be returned to treatment and a relapse prevention plan is implemented and proceeding smoothly.

Detoxification

Immediately upon admission, each veteran is assessed to determine the level of detoxification services are required. When medically indicated, detoxification will be managed on an inpatient basis prior to inpatient/residential treatment. Veterans requiring medically managed detoxification will be sent to an approved medical facility. All veterans must have 72 hours of monitored abstinence prior to readmission to the Palms treatment program.
